The Medicaid excess profit fund first covers losses required by law. After that, the fund can pay for care and system fixes under Medicaid, fill service gaps, and support home visiting and medical respite. It can also pay for translation, continuous glucose monitors, prenatal supports, intergenerational care grants, and other services that keep access to care. The Legislature can allow the fund to reimburse the actual costs of eligible aging services.
The Department pays each area agency on aging 75% of its actual cost for eligible services. Payments are made by the 20th of each month. Costs above the department‑approved budget are not paid, and payments are cut proportionally if total requests exceed available funds. The law also states the Legislature’s intent to appropriate $2 million in FY2025‑26 and $2 million in FY2026‑27 from the Medicaid excess profit fund, split equally among eight agencies ($250,000 per agency each year), to add eligible services.
Starting October 1, 2024, interest and investment earnings from the Medicaid excess profit fund go to the state’s General Fund. This moves investment income away from the fund’s programs and into the state’s main budget.
