More Medicaid access and provider pay

The law creates the Medicaid Access and Quality Fund. Starting January 1, 2026, $40 million a year raises Medicaid rates for nonhospital physical health providers. It focuses on evaluation and management, labor and delivery, and rural care. These dollars cannot replace existing state funds. Starting January 1, 2027, qualifying primary care medical homes get at least $75 per member each month. Medicaid practitioner fee schedule rates and state support cannot go below July 1, 2024 levels. The Department must file federal plan changes by August 1, 2025 and may set rules. Any leftover fund money goes to Medicaid and CHIP. All uses require federal approval.

New 6% premium tax on HMOs

Beginning January 1, 2026, HMOs pay a 6% tax on non‑Medicare premiums written in Nebraska. This tax is on top of the state's general insurance premium tax. Capitation payments under the Medical Assistance Act are not taxed. Prepaid limited health service organizations are taxed the same and get the same credits. All tax revenue goes to the Medicaid Access and Quality Fund.
