The law creates a statewide program to help youth return home from group care or probation. A “youth” is under 19 and leaving a congregate placement or probation. DHHS supervises youth in its custody; Probation supervises youth on probation. After reentry, the right agency provides supervision to support the transition. The program starts December 1, 2026.
If a youth qualifies for the Bridge to Independence program, agencies must refer them. Services under this law cannot duplicate that program. This starts December 1, 2026.
Each eligible youth gets a written transition plan at admission. The plan covers health and Medicaid continuity, school, jobs, housing, IDs, benefits, mentors, and family engagement. Youth, family, facility staff, and the agency create it using validated assessments. Check the plan weekly in detention or short stays; check monthly in other placements. In the first 90 days after return, hold a family team meeting at least every 30 days. These rules begin December 1, 2026.
Agencies must run this program with current staff and budgets. The law authorizes no new General Fund money. Agencies can use existing federal or private funds. Services are not required after probation ends or after a youth leaves DHHS custody. These limits take effect December 1, 2026.
The law defines “credible messenger” mentors as trained people with lived foster care or justice experience. Agencies may set joint rules and minimum training standards for mentors and staff. Effective December 1, 2026.
