Beginning November 1, 2025, Oklahoma patients can get physical therapy evaluation and treatment without a referral for up to 30 days. This includes licensed therapists working under direct-to-employer contracts, but excludes workers’ compensation claims. Physical therapist assistants can provide treatment under a licensed physical therapist’s supervision. After 30 days, treatment requires a referral from a physician with an unlimited license or that physician’s physician assistant. Dentists, chiropractors, podiatrists, and advanced practice registered nurses can also refer patients within their training and practice. Children receiving physical therapy under federal special education and disability laws can receive services without a physician referral. These laws are IDEA 2004 and Section 504 of the Rehabilitation Act. A care plan developed by someone authorized under Oklahoma’s Physical Therapy Practice Act counts as a prescription for these services. Therapists can also provide screening, education, and fitness, wellness, or prevention services unrelated to treating an injury or ailment without physician referral. The law preserves nontherapeutic baths, massage, and normal exercise. It allows incidental work by students in Oklahoma-approved physical therapy programs at recognized schools whose practitioners are licensed. Federal employees are exempt while performing official duties, but must be licensed for work outside those duties. The law does not authorize therapists or assistants to practice other healing professions, and violations remain punishable as misdemeanors.
