No-cost breast imaging for insured Pennsylvanians
Covered Pennsylvania health policies must cover mammograms without deductibles, copays, or coinsurance for people age 40 or older. People under 40 qualify with a doctor's recommendation. The law also requires no-cost supplemental screenings and diagnostic breast exams for people with at least average breast-cancer risk. These exams must be medically necessary and clinically appropriate. Supplemental screenings use standard or abbreviated MRI or ultrasound, with a treating doctor's recommendation when no abnormality is suspected. Diagnostic exams use mammography, MRI, or ultrasound when an abnormality is seen or suspected. Insurers must verify federal certification of mammography facilities before paying. Coverage remains subject to federal law, policy terms, and permitted medical review; imaging beyond the minimum can carry patient costs. The law does not require mastectomy coverage. The act takes effect 60 days after enactment. Requirements apply to policies with rate or form filings on or after 180 days after that effective date. When no filing is required, they apply to policies issued or renewed on or after that point.
