Starting with the 2025–2026 school year, Texas makes school vaccine exemption forms easier to get. The Texas Department of State Health Services must post a printable blank form online. You can also request one in writing without providing information to obtain it. The law removes requirements for seals and other features that prevent copying. The form must state that the person understands the benefits and risks of vaccination and not being vaccinated. For minors, that statement applies to the parent, managing conservator, or guardian. The department cannot keep identifying information about anyone who downloads, accesses, requests, or submits a form. It may count online access and requests; required tracking of forms sent and yearly legislative reports are removed. The department must update the form within 30 days after the law takes effect. The effective-date rule specifies immediate effect with two-thirds support from all elected members of each chamber; otherwise, September 1, 2025.
