Beginning June 1, 2026, Texas local governments must offer their firefighters free, confidential occupational cancer screenings. The first screening is in the firefighter’s fifth year of employment, followed by screenings each year. Screenings include cancer testing, urine and blood tests, lung function testing, an electrocardiogram, infectious disease screening, and breast cancer screening. A chest x-ray is available once every five years. Local governments are exempt if they provide annual occupational medical exams under a physician-endorsed plan that substantially follows National Fire Protection Association standards. They must submit that plan to the Texas Commission on Fire Protection by February 1 each year. The commission must set minimum screening standards using the association’s standards and adopt rules for the annual-exam alternative.
