Form and contents of certificate.

Sec. 13. A commercial housing facilities exemption certificate shall be in the form the commission determines and shall contain: (a) A legal description of the real property on which the new facility is to be located. (b) A statement that unless revoked as provided in this act the commercial housing facilities exemption certificate shall remain in effect for the period stated in the certificate.
