§ 5604. Arbitration forms, brochures, and materials. The arbitration\nadministrator shall submit to the superintendent, for review and\napproval, required forms of agreement and explanatory brochures and\nmaterials to be used for the arbitrations authorized by section\nforty-four hundred six-a of the public health law and article\nseventy-five-A of the civil practice law and rules.\n
