License - Issuance

The board shall cause to be issued to each pharmacist in this state whom it finds entitled thereto, a license showing: 1. The date of issue. 2. The fact that the person to whom it was issued is a licensed pharmacist. 3. The residence of the person to whom the license was issued. The license must be signed by a majority of the members of the board.
