Duty to report change of name or contact information. A physical therapist or physical therapist assistant shall report to the Oregon Board of Physical Therapy a change to the physical therapist’s or physical therapist assistant’s name, business or residential address, contact email address or contact telephone number within 30 days after the date of the change.
