(1) The application of a foreign PLLC for a certificate of authority in this state shall:(1) Contain the information required in § 48-249-904;(2) State that it is a foreign PLLC;(3) State that its purpose is to render specified professional services; and(4) Include a statement that the requirements of § 48-249-1123(b)(3) are satisfied.
