In the event the licensee desires to cancel the license, he shall notify the Commissioner and the Commissioner shall supply him with proper forms as designed by the Commissioner to effectuate the cancellation of said licensee. Upon cancellation of said license, the licensee shall forthwith return to the Commissioner the license so cancelled.
