A person who desires to be licensed as a speech-language pathologist or audiologist or certified as a speech language pathology assistant or audiology assistant shall apply to the Board in writing on an application form prescribed by the Board. At the time of filing the application, an applicant for licensure or certification shall pay an application fee to the Board.
