    In the spring of 2022, hospitals across the United States suddenly had to ration CT scans — not for lack of scanners, but for lack of dye. GE HealthCare makes Omnipaque (iohexol), the iodine-based contrast agent injected into patients to make blood vessels, organs and tumors visible on CT and angiography, and the company is the market leader. A very large share of US supply came from a single manufacturing plant in Shanghai. When Shanghai went into a strict COVID-19 lockdown, that plant's output was disrupted, and within weeks the US faced a severe shortage of CT contrast media. Hospitals postponed non-urgent imaging, switched protocols, and triaged who got contrast scans for strokes, cancers and trauma. It was a stark illustration that even high-tech medical care can be hostage to one factory in one city: the bottleneck wasn't the imaging machine or the radiologist, but the humble dye, made an ocean away under another country's pandemic policy.

    Iodinated contrast media is a doubly concentrated chokepoint. The finished agents come from just a few makers — GE HealthCare (Omnipaque), Bracco, Guerbet and Bayer — and they all depend on a single scarce raw material: iodine, most of which is mined or extracted in only two places on Earth, Chile (from caliche ore) and Japan (from gas-field brine). So the dye that makes modern diagnostic imaging possible rests on a handful of pharmaceutical manufacturers sitting atop an iodine supply concentrated in two countries. A disruption at either layer — a contrast plant (as in 2022) or iodine supply — propagates into the ability to perform CT angiography, cancer staging and stroke workups worldwide. It's the same pattern as sterile generics and chemo drugs: a medically indispensable product whose supply chain narrows, at one or more points, to very few sources, leaving the whole system one shock away from shortage.

    GE HealthCare straddles two very different supply chains for the same goal — seeing inside the body — and both have hidden chokepoints. On one side it makes the consumable: contrast dyes and radiopharmaceutical tracers.
