TAPI's azithromycin API illustrates how a single generic drug's supply chain connects to multiple policy contexts simultaneously: (1) Bacterial infection treatment — azithromycin Z-pack is the standard treatment for community-acquired pneumonia, Chlamydia, and other bacterial infections; it is one of the most prescribed antibiotics in the US and globally; (2) COVID-19 unproven treatment — in March-April 2020, hydroxychloroquine + azithromycin became the most sought-after drug combination despite no clinical evidence; US stockpiling created a global azithromycin shortage; TAPI as world's largest supplier faced unprecedented demand pressure for an antibiotic from a viral pandemic it couldn't treat; (3) Antibiotic stewardship concern — azithromycin's overuse (as Z-packs were aggressively prescribed and patients rarely completed full courses) has been identified as a major driver of macrolide-resistant respiratory bacteria globally; TAPI as dominant supplier is structurally embedded in both the treatment of bacterial infections AND the antibiotic resistance crisis created by azithromycin overuse. The world's largest azithromycin supplier is simultaneously a critical healthcare resource AND a supplier to the pathway that created macrolide resistance.

  • Origin2023

TAPI (Teva API) is the dedicated API business of Teva Pharmaceutical Industries — the world's largest generic drug company, headquartered in Petah Tikva, Israel. Teva was founded in 1901 in Jerusalem as a small wholesale drug distributor; the generic business grew through Israel's unique pharmaceutical ecosystem (Israeli army's need for self-sufficient drug supply during wars; Israel's small domestic market forcing international ambition). Teva became the dominant global generics company through a series of acquisitions including Barr Pharmaceuticals (2008), Ratiopharm (2010), and Allergan Generics (2016) for USD 40.5 billion — the Allergan acquisition, made at the peak of pharma deal valuations, proved catastrophic: drug price deflation in the US collapsed Teva's revenue shortly after the deal closed; the company's stock fell ~80% in 2017; CEO left; massive layoffs and restructuring followed. Teva also became the largest single defendant in the national US opioid litigation (settling for USD 4.25 billion in 2023 for its role in supplying oxycodone and other opioids). The largest generic drug company in the world nearly collapsed from two simultaneous crises: market price deflation AND opioid liability.
