Creates and supports demonstration projects to try new ways of delivering health care to American Indians and Alaska Natives. The Secretary, through the Indian Health Service, or Indian tribes and tribal organizations working under contracts or compacts under the Indian Self-Determination and Education Assistance Act (25 U.S.C. 450 et seq.), may run projects that test alternative facility-based care or new care models (including primary care, contract health services, convenient care services, community health centers, and cooperative arrangements with other providers). The Secretary may approve contracts to build or renovate hospitals and clinics, waive leasing limits, allow carryover and use of Service funds (including for buying health benefits as authorized by section 1642(a)), allow other Federal and non‑Federal funds and third‑party collections (per sections 1621e, 1621f, and 1641), accept donated funds or property (and return donated property to donors), and use Service funds to match other funds. Projects must meet criteria: a clear need for a new or changed program or facility; serve a significant number of Indians (including those with poor health); be efficient, effective, and economically viable; and, for tribal projects, show administrative and financial capacity. Projects must coordinate with other health and social services and avoid duplication. The Secretary will give priority to projects in Cass Lake, MN; Mescalero, NM; Owyhee and Elko, NV; Schurz, NV; and Ft. Yuma, CA. The Secretary may approve up to 10 additional projects each fiscal year that address lack of access (for example limited hours) and that expand services or reduce pressure on Contract Health Services or emergency rooms. The Secretary must offer technical help to applicants, may include services to otherwise ineligible persons and non‑Service practitioner privileges subject to section 1680c, and must evaluate and integrate contract/compact facilities using the same standards as Service facilities.
