Information required by sections 11131, 11132(a), and 11133 must be sent on a regular schedule — at least once a month — in the form and way the Secretary decides. Reporting must start on a date the Secretary sets, no later than one year after November 14, 1986. The reports go to the Secretary or, if the Secretary chooses, to a public or private agency that has an agreement with the Secretary to receive, store, protect, and share the information. Reports under 11131 also go to the state licensing board where the malpractice claim happened. Reports under 11133(b) must go to the licensing board in the state where the health care entity is located if they were not already sent.
