Title 42 › CHAPTER 6A — PUBLIC HEALTH SERVICE › SUBCHAPTER II — GENERAL POWERS AND DUTIES › Part D — Primary Health Care › subpart v — healthy communities access program › § 256a
The Secretary, through the HRSA Administrator, can give grants to eligible health organizations to set up programs that hire, train, and place patient navigators who know the communities they serve. The Secretary must work with the Indian Health Service, the National Cancer Institute, the Office of Rural Health Policy, and other appropriate federal offices when designing and evaluating the programs. Patient navigators must help patients get and coordinate care, make referrals, involve community groups, tell people about clinical trials and help them enroll if they want, find and fix barriers to quick diagnosis and treatment, share information about insurance and public benefits, and do outreach to underserved groups. Navigators may not take referral fees or kickbacks. Grant money cannot pay legal fees from lawsuits or similar proceedings. Grants can run up to 3 years and may be extended up to 1 year, but no grant can last more than 4 years total. Applicants must explain how they will measure results, prove navigators meet basic skills, and show grants will add new or expanded services if they already get federal funds. The Secretary will set common baseline measures, give preference to projects that tackle big local barriers, coordinate with other programs, and require reports. The Secretary must study the results and send Congress a report within 6 months after the program ends. Eligible entities include certain public or nonprofit health centers, Indian Health Service facilities, hospitals, cancer centers, rural clinics, academic centers, and similar nonprofit partners. A “patient navigator” is a person who finished Secretary-approved training. Funding authorized: $2,000,000 for FY2006; $5,000,000 for FY2007; $8,000,000 for FY2008; $6,500,000 for FY2009; $3,500,000 for FY2010; and whatever sums are needed for FY2011–FY2015. These amounts remain available through the end of FY2015.
