An employee hurt on the job, or someone for them, must give written notice within 30 days. For a death tied to the job, an eligible beneficiary named in section 8133, or someone for them, must give notice to the employee’s immediate superior by personal delivery or by mail properly stamped and addressed. The signed notice must include the employee’s name and address; the year, month, day, hour, and place of the injury or death; a description of the injury or the work factors believed to have caused it; and the sender’s address.
