Military Family Special‑Needs Screening

If you are an active‑duty service member requesting command sponsorship for accompanied travel to remote or outside the continental United States, you (or your family) must use DD Forms 3040, 3040‑1, 3040‑2, 3040‑3, and 3040‑4 to document any special medical, dental, or educational needs. The collection covers 302,205 respondents and is required by the National Defense Authorization Act of 2010 and DoDI 1315.19 (the Exceptional Family Member Program) to verify whether necessary services are available at the gaining installation.

Time Burden to Complete Screening Forms

Filling out each Family Member Travel Screening response takes an average of 18.026 minutes. With 302,205 annual responses, the DoD estimates 90,793 total annual burden hours; public comments on the collection are due by February 5, 2025.
