You must complete DD Form 2875 (System Authorization Access Request) when you request access to a Department of Defense information system, application, or database. The form collects background investigation and clearance information under Executive Order 10450 and is validated by a cognizant Security Manager to ensure access is consistent with national security. DoD estimates 900,000 respondents, 7,200,000 annual responses, 600,000 annual burden hours, an average burden of 5 minutes per response, and requests comments by March 7, 2025.
