New Accommodation Request Form

The Merit Systems Protection Board (MSPB) will submit a new Accommodation Request Form (OMB Control No. 3124-0NEW) to streamline requests for reasonable accommodations from MSPB employees, job applicants, and participants in MSPB programs. The form covers disability, medical, pregnancy-related, and religious accommodation requests and is intended to reduce paperwork and respondent burden.

Sensitive Documentation Will Be Collected

MSPB's new collection explicitly includes supporting documentation such as medical records, doctor's notes, and documentation of sincerely held religious beliefs; these records, evaluations, and the decision about the request are part of the collection and will be maintained and used by MSPB.
