NRC forms renewal keeps reporting requirement

If you hold an NRC license (commercial nuclear power plant, fuel cycle facility, NRC material licensee, or non‑power reactor), the NRC is renewing OMB approval (OMB No. 3150-0238) for Forms 361, 361A, and 361N as an extension. The collection is estimated at 563 annual responses, 2,105 annual respondents, and 281.5 hours annually; comments are due by June 22, 2026.

Forms serve as telephone reporting worksheets

NRC Forms 361, 361A, and 361N are worksheets licensees are not required to fill out or submit, but they show the order of information the NRC will request during telephonic event notifications. Using the forms helps licensees provide required information more efficiently and the NRC says lacking the form format could cause delays in completing notifications that have potential impact to public health and safety.
