$25M Fund for Small Rural Hospitals
If you run a small rural hospital, HRSA will make $25,000,000 available in FY 2026 through the Rural Hospital Provider Assistance Program to pay eligible hospitals directly. The program is authorized by the Consolidated Appropriations Act, 2026 and will be administered by HRSA's Federal Office of Rural Health Policy.
Who Qualifies — Size, Wages, Rural Location
To get payments, a hospital must have no more than 50 acute care inpatient beds, have an established wage index value less than 0.90, and be located in a HRSA-designated rural area. These eligibility criteria come from the Consolidated Appropriations Act, 2026.
How Payments Are Calculated
Payment amounts will be determined by dividing the total $25,000,000 equally among eligible hospitals. Up to 10 percent of each hospital's award may be used for administrative expenses. HRSA preliminarily identified 167 eligible hospitals and estimated approximately $148,000 per hospital assuming all apply, with the final award adjusted based on actual applicants.
Application Deadline and Submission
If you want funding, your hospital must submit a completed application electronically through Grants.gov by July 1, 2026, at 11:59 p.m. Eastern Time. Applicants are strongly encouraged to register with SAM.gov and Grants.gov as soon as possible because registration can take several weeks.
Exact Data Used to Determine Eligibility
HRSA will determine inpatient bed counts using acute care beds reported in the FY 2025 Medicare cost report (CMS HCRIS Quarterly Update as of December 31, 2025, Worksheet S-3, Part I, Line 14, column 2). Wage index values will come from Table 2 in the FY 2026 Hospital Inpatient Prospective Payment System final rule as published by CMS.
