A grant program to treat diabetes in medically underserved urban and rural communities. This bill would create funding for providers to deliver routine diabetes care, manage complications like kidney, eye, and foot disease, and run culturally and linguistically appropriate education and outreach.

- Families and patients in medically underserved areas would gain expanded access to routine diabetes services, education, eye and foot care, and kidney disease treatment.
- Community-based organizations, public or nonprofit health providers, Federally Qualified Health Centers (FQHCs), rural health clinics, and State, local, or Tribal health departments would be eligible to apply for grants.
- Grantees must offer services in languages and cultural formats suited to the population and conduct outreach so communities know about available services.
- The Secretary must aim for an equitable geographic distribution and balance urban and rural needs. The program would be funded by annual appropriations for FY2026 through FY2031.

Diabetes care grants in underserved communities
