  • Families and patients in medically underserved areas would gain expanded access to routine diabetes services, education, eye and foot care, and kidney disease treatment.
  • Community-based organizations, public or nonprofit health providers, Federally Qualified Health Centers (FQHCs), rural health clinics, and State, local, or Tribal health departments would be eligible to apply for grants.
  • Grantees must offer services in languages and cultural formats suited to the population and conduct outreach so communities know about available services.
  • The Secretary must aim for an equitable geographic distribution and balance urban and rural needs. The program would be funded by annual appropriations for FY2026 through FY2031.
