- People at risk: Individuals in high‑incidence communities, including rural and uninsured populations, could receive PrEP medications, testing, counseling, and transportation without out‑of‑pocket costs.
- Grantees: States, Indian Tribal governments, Federally qualified health centers, rural clinics, hospital and university clinics, and community organizations can apply for grants up to $10.0 million each. Grants generally require a 10% match but Tribal governments are exempt and waivers may be available for some clinics.
- Data and oversight: The Department of Health and Human Services must publish annual reports for five years with disaggregated data by race, gender identity, age, and location and evaluate changes in disparities and PrEP uptake.

Free PrEP care for uninsured
If enacted, this bill would require HHS to set up, within 1 year, a program that lets registered providers submit claims for PrEP-related items and services they give to uninsured people. Subject to available appropriations, HHS would pay providers set amounts and review those amounts at least every two years. Clinical diagnostic lab tests would be paid at Medicare Part B rates. HHS would also provide a "PrEP Pass" card or technology so uninsured people can get covered services at no cost, and participating providers would agree not to bill the person for costs paid under the program.
Grants to expand no-cost PrEP
If enacted, this bill would require HHS to set up a grant program within 1 year to fund local PrEP programs. It would authorize $400 million for each fiscal year 2027 through 2031 and cap any single grant at $10 million. Most grantees would need to provide at least 10% matching funds, with exceptions for Indian Tribal governments and possible waivers for certain clinics. Grant funds would pay for clinic and lab fees, office and telehealth visits, PrEP medication and testing, outreach, peer navigation, transportation, mental health services, and related costs. People who get services from programs funded by these grants would not be required to pay out of pocket, and HHS would report disaggregated results to Congress each year for five years.
