Opt in to child passport alerts
You (a legal parent or guardian) can ask to join the Children's Passport Issuance Alert Program (CPIAP) to be notified if the Department of State receives a passport application in your child's name. The request uses Form DS-3077 and is voluntary and free.
Passport applications may be held
If a minor's name is entered into CPIAP and the Department receives a passport application for that minor, the application may be placed on hold for up to 90 days while the Office of Children's Issues attempts to notify the requestor. This hold can delay issuance or travel plans for the applicant.
Consular Report of Birth no longer sufficient
The DS-3077 instructions remove the option to use a Consular Report of Birth Abroad (CRBA) alone as evidence of parental relationship, sole parentage, or custody. Requestors must provide other documentation to show parentage, guardianship, legal custody, or authority to act on behalf of a parent or guardian.
Form modernization and e-sign option
The updated Form DS-3077 will allow a typed e-signature, permit a web-based online version, and removes fax as a transmittal option. The Department estimates the form takes about 30 minutes to complete and the collection is voluntary.
