If you ask the FOIA Ombuds (OGIS) for help with a Freedom of Information Act request or appeal, you may need to sign NA Form 10003 to give written consent before agencies can share records about your request. The Privacy Act requires this prior written consent unless an agency already has a routine use allowing release to OGIS.

Consent form already digital and may go online

NA Form 10003 is available as a fillable PDF with an electronic signature option so you can email a signed copy or print and mail it. OGIS says it cannot yet collect consent via an online case management system but hopes to add online submission technology in the future, which would make intake and consent submission smoother.
