Summary
The Department of Labor (DOL), as part of its continuing effort to reduce paperwork and respondent burden, conducts a pre- clearance consultation program for all information collections, to provide the public and Federal agencies with an opportunity to comment on proposed collections of information, in accordance with the Paperwork Reduction Act of 1995. This program helps to ensure that requested data can be provided in the desired format, reporting burden (time and financial resources) is minimized, collection instruments are clearly understood, and the impact of collection requirements on respondents can be properly assessed. The Mine Safety and Health Administration (MSHA) is soliciting comments on the information collection titled "Mine Accident, Injury and Illness Report and Quarterly Mine Employment and Coal Production Report."

Immediate 15‑Minute Accident Notification
If you operate a mine, you must immediately contact MSHA without delay and within 15 minutes at the toll‑free number once you know or should know an accident occurred involving: (a) a death, (b) an injury with a reasonable potential to cause death, (c) an entrapment with a reasonable potential to cause death, or (d) any other accident (30 CFR 50.10).
Form Filing Deadlines and 5‑Year Records
If you operate a mine, you must mail completed MSHA Form 7000‑1 within 10 working days after an accident, occupational injury, or diagnosis of an occupational illness, and submit MSHA Form 7000‑2 within 15 days after the end of each calendar quarter. You must keep copies of investigation reports, Form 7000‑1, and Form 7000‑2 at the mine office for 5 years and, if more than one miner is affected, complete a separate set of forms for each miner.
