Specimen Submission Forms Burden

If you are a State, local, or tribal veterinarian, accredited veterinarian, private laboratory, research institution, owner, or producer who submits animal specimens (blood, milk, tissue, urine, or ticks) to APHIS' National Veterinary Services Laboratories, you must use VS Forms 10-4 and 10-4A (Specimen Submission) or VS Form 5-38 (Parasite Submission).
