Specimen Submission Forms Burden
If you are a State, local, or tribal veterinarian, accredited veterinarian, private laboratory, research institution, owner, or producer who submits animal specimens (blood, milk, tissue, urine, or ticks) to APHIS' National Veterinary Services Laboratories, you must use VS Forms 10-4 and 10-4A (Specimen Submission) or VS Form 5-38 (Parasite Submission). The collection covers 14,860 respondents with a total estimated burden of 30,930 hours and occurs on occasion.
