Mandatory Radiation Monitoring Form

If you work with or near ionizing radiation on the NIST campus (including federal employees, visitors, contractors, and associates), you must complete Form NIST-366A when required. The agency estimates 600 respondents, about 15 minutes per response, and a total burden of 150 hours; the collection is mandatory under 10 CFR 20.1502.

Exposure Records and Reporting Rights

NIST will use information from Form NIST-366A to maintain individual radiation exposure records and to provide reports to monitored individuals on request and to the NRC annually, as required by 10 CFR 20.2106. The form ensures exposure data are associated with the correct person using unique identifiers.
