ARC Act of 2025
Sponsored By: Representative McIver, LaMonica [D-NJ-10]
Introduced
Summary
Prevent major lower‑limb amputations by expanding screening, education, and coordinated care for people at high risk of peripheral artery disease (PAD). This bill would require Medicare and Medicaid to cover standardized PAD screening with no cost‑sharing, fund a national education program, and push quality measures and a Center for Medicare and Medicaid Innovation pilot to reduce amputations.

Bill Overview
Analyzed Economic Effects
5 provisions identified: 5 benefits, 0 costs, 0 mixed.
New amputation quality measures for Medicare
If enacted, the HHS Secretary would develop and validate quality measures aimed at reducing nontraumatic lower‑limb major amputations. Measures must include diagnostic screening such as PAD screening and encourage alternatives to amputation like revascularization. Validated measures would be tested and added into Medicare reporting and payment programs (for example MIPS, eligible APM incentives, the Shared Savings Program, and CMMI models) within 18 months after enactment.
Voluntary amputation prevention pilot program
If enacted, the Center for Medicare and Medicaid Innovation (CMMI) would test a voluntary Amputation Prevention model at hospitals, ambulatory surgical centers, and office‑based centers. The model would focus on risk management, early screening and surveillance, PAD testing and treatment, and better care coordination to increase access and reduce amputations and facility costs. CMMI must start testing the model not later than 18 months after enactment. Participation by providers would be voluntary.
Free PAD screening for Medicaid
If enacted, State Medicaid plans and waivers would have to cover PAD screening tests for defined at‑risk enrollees. Covered tests would include ankle‑brachial index testing, arterial duplex scans, and other items the Secretary approves. States would not be allowed to charge cost‑sharing for these covered PAD screening tests, and coverage rules would follow Secretary-established frequency standards. Coverage would apply to items furnished on or after January 1, 2026.
Free PAD screening for Medicare
If enacted, Medicare would cover peripheral artery disease (PAD) screening tests for "at-risk" beneficiaries. Covered tests would include ankle‑brachial index tests, arterial duplex scans, and other tests the HHS Secretary designates. Medicare would pay 100% of the lesser of the provider's charge or the section 1848 payment amount and the Part A/B deductible would not apply. Coverage would start for items furnished on or after January 1, 2026, and tests given more often than Secretary-established standards would not be covered.
National PAD education program funding
If enacted, HHS (through the CDC and in coordination with CMS, HRSA, clinical groups, and patient advocates) would establish a national program to educate clinicians and the public about PAD and ways to prevent amputations. The bill would authorize $6,000,000 each year for fiscal years 2026 through 2030 to carry out the program.
