Apples to Apples Comparison Act of 2025

Sponsored By: Representative Bean, Aaron [R-FL-4]

In Committee

Summary

Medicare expenditure transparency would require CMS to publish monthly, county- and MSA-level Medicare spending broken down by a detailed set of enrollment categories. It would also force public MA versus fee-for-service analyses from MedPAC and add disaggregated spending in the Trustees' reports.

Show full summary
  • Beneficiaries and local leaders would see total and average Medicare spending by county and MSA for each month, with data split into many enrollment types including Parts A, B, D, Medicare Advantage, MA-PD, employer plans, and supplemental policies.
  • Researchers and states would get machine-readable files and underlying data and methods. The 2025 publication must include enrollment breakdowns back to 2015 to allow year-by-year local comparisons.
  • CMS, the Medicare Payment Advisory Commission, and the Boards of Trustees would face new reporting and transparency duties. CMS must publish the files within 30 days after year end, MedPAC would annually compare MA and fee-for-service spending and publish methods with at least 30 days for public comment, and the Trustees must add disaggregated Part A and B expenditure information starting in 2026.
