Published Date: 12/11/2025

Notice

Summary

The Agency for Healthcare Research and Quality wants to keep collecting info using their Patient Safety Culture Hospital Survey, which helps hospitals understand safety and errors. They’re asking for 30 more days of public comments before getting final approval, with no changes to the survey. Hospitals and healthcare workers are the main players here, and there’s no new cost or big time changes involved.

Time and Cost Burden for Data Submitters

AHRQ estimates that 165 hospital points-of-contact (POCs) will perform the submission steps annually, totaling 222 burden hours and an estimated annual cost burden of $31,880. Typical tasks include a 3-minute Eligibility/Registration form, three 5-minute Site Information forms, a 3-minute Data Use Agreement, and about 1 hour on average to submit SOPS Hospital Survey data files.

Hospitals Keep Voluntary Safety Survey

AHRQ will reinstate, without change, the Surveys on Patient Safety Culture (SOPS) Hospital Database (OMB No. 0935-0162) so U.S. hospitals can voluntarily submit SOPS Hospital Survey 2.0 data (40 items) to the database. Hospitals or their vendors that submit data receive a feedback report and an aggregated, de-identified report of other hospitals' findings to help identify strengths and areas for improvement in patient safety culture.
