Reinstated Radiation-Exposure Claim Form

You can use the reinstated DOJ form titled "Claim for Damage, Injury, or Death" (CIVSF 95) to apply for compensation under the Radiation Exposure Compensation Act and the Federal Tort Claims Act. The form is being reinstated without change and filing is voluntary but required if you want to obtain the benefit (compensation).
