Provide Forms to Get Casualty Benefits

If you are the Next of Kin or a designated beneficiary of a Sailor who is deceased, missing, or seriously ill or injured, you must provide information on OPNAV Forms 1770/1, 1770/2, and 1770/3 (OMB Control Number 0703-0076) so the Navy can process Death Gratuity, Service Members' Group Life Insurance, and Survivor Benefit Plan claims. The forms are also used to arrange travel under 37 U.S.C. 452 and to comply with legal payment authorities such as 10 U.S.C. 1475.

Paperwork Time Burden on Families

Completing the Navy casualty assistance forms imposes a time burden on affected family members: there are 800 respondents, 2,000 annual responses, an average burden of 39 minutes per response, 2.5 responses per respondent, and an estimated 1,300 annual burden hours. The collection occurs on occasion when casualty assistance is required.
