For full casketed remains, funeral homes and the receiving Navy port must use OPNAV Form 5360/2, “Burial at Sea Port Checklist,” to ensure the casket is prepared, transported, and disposed of in compliance with law. The Navy estimates 20 respondents annually for this form, with an average burden of 90 minutes per response and 30 annual burden hours in total. The collection is reinstated under OMB Control Number 0703-0082 and the agency seeks comments by April 13, 2026.
