Researchers Must Sign Data Security Forms

If you are a researcher approved to use restricted NCHS microdata, you must complete and sign data security forms before access. Required forms include the Data Use Agreement Form and the Designated Agent Agreement Form for Research Data Center access, and those plus the VDE Data Use Agreement Form for access through the NCHS Virtual Data Enclave.

Estimated Time Burden and Cost to Researchers

CDC estimates the data security forms will take about 1 hour per researcher and totals 110 hours annually (330 hours over the three-year clearance). There is no monetary cost to researchers except time, unless a researcher incurs a nominal notary fee.
