Within one year of the section's operative date, plans and insurers must follow a 90-day credentialing deadline. The 90 days start after they get a complete application, including all third-party checks. They must confirm receipt and say if it is complete within 10 business days. If they approve before day 90, they must activate you and notify you within 10 days. If they miss 90 days, your credentials are provisionally approved for 120 days. This does not apply if you have licensing discipline, adverse actions or malpractice payments in the National Practitioner Data Bank, or were not credentialed by that plan or insurer in the past five years. The 90-day clock covers credentialing only, not contracting. These rules do not apply to some State Department of Health Care Services contracts.
Beginning January 1, 2028, full-service health plans and health insurers must use the CAQH credentialing form and process. They may only ask for extra details to clarify or confirm what you put on the form. They must keep extra requests to a minimum. You must answer any extra request within 10 business days and keep your CAQH profile current. These rules do not apply to some contracts with the State Department of Health Care Services.
