Direct referrals and coordination for homeless Medi-Cal

If you are experiencing homelessness and see a field medicine provider, they can directly refer you for covered services. This includes specialists, tests, medicines, medical equipment, and transportation. Your plan or IPA must set up referral and authorization systems and make sure providers share information quickly. These steps help coordinate care and avoid duplicate services, subject to federal approvals and funding.

Medi-Cal care outside clinics for homeless

The law creates Medi-Cal field medicine. Plans can choose to use in-network field medicine providers who treat people outside clinics. If your plan offers this and you are experiencing homelessness, you can get care from that provider even if your assigned primary care doctor or IPA is different. This works alongside other Medi-Cal services and applies with required federal approvals and funding.

Medi-Cal pays field providers in fee-for-service

For beneficiaries experiencing homelessness in fee-for-service Medi-Cal, the Department pays enrolled field medicine providers for covered care they deliver. This creates a payment path for care outside clinics. It applies with required federal approvals and federal funding.

Easier to tell Medi-Cal you are homeless

Your Medi-Cal plan must let you report that you are experiencing homelessness online, in person, or by phone. The Department also tells your plan if you marked homelessness on your Medi-Cal application. Beginning January 1, 2027, the standard application adds an optional question so you can identify homelessness. The plan and department steps apply with required federal approvals and funding.
