911 callers get medical instructions

Beginning January 1, 2027, 911 centers that handle medical calls must give step‑by‑step care before help arrives. Instructions include airway and choking help for infants, children, and adults. They also cover AED use and CPR for children and adults, childbirth, and bleeding control. Guidance includes using an epinephrine auto‑injector for suspected anaphylaxis and naloxone for suspected overdoses. An agency may meet this by contracting with another public safety agency that already provides these instructions. A local EMS medical director must approve the instructions, and agencies use them under their medical protocols. Agencies with already approved instructions do not need to change them because of this law.

When state reimburses local costs

The law sets how the state pays for local costs. The state does not reimburse costs tied to creating, changing, or ending crimes or penalties. If the Commission on State Mandates finds other state‑mandated costs, the state must reimburse under Government Code Section 17500 and following.

Exceptions and rules for 911 dispatch

Sending only peace officers to a scene does not count as 911 medical call processing under this law. Those agencies are not required to give prearrival medical instructions. The law does not change the power to set emergency medical dispatch guidelines and training. It also does not override Health and Safety Code 1797.201, 1797.223, 1798.6, 1798.8, or Government Code 53110.
