Special fund to finance hospital training

The law creates a Special Fund in the State Treasury to hold transfers for these hospital training payments. The nonfederal share must come only from voluntary intergovernmental transfers by these hospitals, their affiliates, or other public entities, not the State General Fund. Each year the department sets transfers to cover the nonfederal share plus 5% of that share, calculated as if the federal match rate were 50%. The department must claim federal matching funds and return related transfers within 14 days if federal funds are not available or are taken back. Transfers must meet federal Medicaid rules and be certified by the senders.

New Medi-Cal training payments to public hospitals

The law creates extra Medi-Cal payments to district and municipal public hospitals and their government affiliates for doctor training costs. Payments use the same direct and indirect methods used for other public hospitals. The department must get federal approval and matching funds, and payments take effect no earlier than January 1, 2026. The department can also seek other payment types, like fee‑for‑service volume or incentive payments, if federally approved. The department consults with these hospitals and can use letters or bulletins to run the program.
