Colorado’s health department must create a standard dementia care information form by July 1, 2027, consulting the state long-term care ombudsman. Related rules cannot exceed applicable national standards. Starting October 1, 2027, department-licensed residential dementia care facilities must give the form to care seekers. They must also post it online and keep it on-site for inspection. The form explains staff training, restraint use, security, emergency response, placement, transfers, discharge, family involvement, incident notices, and medical staff availability. It explains fees and fee-change notices, and links to federal Care Compare information about enforcement, complaints, and payment sources. Facilities must review it at least every five years and when information changes, updating it as soon as practical. The department must cite facilities that fail to maintain or produce the current form. The law allows Assisted Living Residence Improvement Cash Fund money and collected civil fines to support the form and related rules. It sets no funding amount.
