For payments owed on or after August 12, 2026, insurers must show their median in-network reimbursement rate on out-of-network payment notices. After payments covered by Colorado’s out-of-network payment rules, providers and facilities can ask the insurance commissioner to collect payment data. The required data includes how insurers set that median rate and reimbursement for each service in the same geographic area. Insurers’ submitted data stays confidential as a trade secret. Beginning January 1, 2027, notices for these covered payments must also identify the plan as Colorado-regulated and name the applicable payment rule.
