Hospitals connect patients to local shelters

The law creates a single Department of Human Services website to help hospitals and health care providers connect homeless or unstably housed patients to shelter and support. It lists contacts for every local continuum of care in Illinois. For each area, the site shows phone numbers, emails, addresses, lead agencies, and, when possible, a direct number and sample words to use. A lookup tool finds the right continuum using the hospital’s address. DHS, with a statewide hospital group, offers a recorded, voluntary training on how to use it. DHS posts the listings by January 31, 2026, and updates them every January 31 and July 31, with audits.
