Summary

Amends the Illinois Insurance Code. In a provision concerning coverage of certain cancer screenings, adds having a high level of CA-125, as indicated by a blood test screening, to the definition of "at risk for ovarian cancer". Provides that "surveillance tests for ovarian cancer" means all medically viable methods for the detection and diagnosis of ovarian cancer, including, but not limited to, ultrasounds, magnetic resonance imagings (MRIs), x-rays, computed tomography (CT) scans, and CA-125 blood test screenings (instead of an annual screening using (i) CA-125 serum tumor marker testing, (ii) transvaginal ultrasound, (iii) pelvic examination). Effective January 1, 2028.

Free ovarian screening for at-risk patients

Beginning January 1, 2028, group health insurance policies in Illinois must cover yearly ovarian cancer surveillance at no cost for people at risk. Covered screening includes a CA-125 blood test, a transvaginal ultrasound, and a pelvic exam, with no deductible, copay, or coinsurance. At risk means certain family histories (like a first-degree relative with ovarian cancer or clusters of breast or colorectal cancer), a BRCA1/BRCA2 mutation, or a high CA-125 level. Plans must also cover medically sound tests to detect or diagnose ovarian cancer for at-risk insureds, such as ultrasound, MRI, CT scans, x-rays, and CA-125. Those diagnostic tests can still have deductibles or copays.

Free yearly prostate screening for eligible adults

Beginning January 1, 2028, group health insurance policies in Illinois cover one prostate cancer screening each year at no cost when your doctor recommends it. This applies if you are age 50 or older and have no symptoms, or if you are African-American and age 40 or older, or if you are age 40 or older with a family history or genetic risk. Screening can include a digital rectal exam, a PSA test with related lab work, and medically necessary follow-up tests like urine tests, blood biomarkers, and imaging.

One free Pap test each year

Beginning January 1, 2028, group health insurance policies in Illinois must cover one cervical smear (Pap) test each year for all insureds. The Pap test is covered with no deductible, copay, coinsurance, or other cost-sharing. This applies to group policies that are amended, delivered, issued, or renewed on or after that date.

Exceptions for limited and high-deductible plans

Beginning January 1, 2028, these screening rules do not apply to policies that only cover a specific disease or that offer other limited benefits. Also, the no-cost prostate screening rule does not apply if offering it would make a high-deductible health plan lose eligibility for a Health Savings Account. This protects HSA tax benefits for those plans but can limit free screening in them.
