HMOs in Illinois must follow many Insurance Code benefit rules. This brings HMO plans under the same required coverage and consumer protections as other insurers. That includes hippotherapy coverage when policies renew after January 1, 2027. The Department of Insurance enforces these rules.
The law requires state, county, city, and school employee health plans to cover post‑mastectomy care and related coverage required by the Illinois Insurance Code. This applies even when a county or city self‑insures. The Department of Insurance enforces the rules for counties, cities, and school plans. For the State Employees Group Insurance Program, the Department of Insurance enforces some rules and the Department of Central Management Services enforces the rest.
Beginning January 1, 2027, group and individual health policies issued, amended, delivered, or renewed after that date must cover medically necessary therapies that use horse movement (hippotherapy). A licensed occupational therapist, physical therapist, or speech‑language pathologist must provide the therapy with a professional horse handler and a therapy horse. Coverage applies only when your therapist finds it medically necessary.
