No surprise bills for ground ambulances
Beginning January 1, 2026, your health plan must pay for covered ground emergency ambulance rides, including 9-1-1 or other unscheduled calls, even if the ambulance is out of network. If a local government set a rate where the ride began, the plan pays that rate; if not, it pays at least 325% of the area’s Medicare rate. That payment counts as full payment, and the ambulance cannot bill you beyond your copay, coinsurance, and deductible. For individual health policies that cover ambulance care, the insurer pays the ambulance company directly. Insurers can still sign contracts with providers; until then, these default payments apply.
No prior approval for emergency ambulance rides
Beginning January 1, 2026, insurers cannot require prior approval for emergency or other unscheduled ground ambulance trips. This covers rides to a hospital, between hospitals, or from a hospital to a nursing home, hospice, or other facility. If local dispatch, a clinician, or a first responder requests the ambulance under local protocols, the trip is treated as medically necessary for coverage.
Faster payments and clearer claim notices
Beginning January 1, 2026, insurers must pay non-participating ground ambulance providers on the state’s prompt-payment schedule. All explanation of benefits notices must clearly say whether the claim is under New Hampshire insurance oversight. This helps you see which protections and complaint paths apply.
Air ambulances not covered by new rules
Beginning January 1, 2026, these ground ambulance protections do not apply to air ambulances. Air ambulance claims are outside these payment and billing rules.
