Workers pay at most $30 for FMLA forms

Nevada health care providers cannot charge more than $30 to fill out an employer-required FMLA certification form. The $30 limit applies per form. Each year, the Nevada Department of Health and Human Services adjusts the cap by the prior year’s Consumer Price Index (CPI), posts it by January 1, and it takes effect January 1. The CPI update can raise or lower the cap.
